    Registration Requirements:

    1. Birth Certificate (Certified Copy)

    2. A copy of the child’s record from the last school attended

    3. Immunization Record

    4. Proof of Residence: Gas, Water, or Electric Bill (No telephone bill or earnest money loan contracts).

    5. Legal Guardianship or Power of Attorney

    6. Social Security Card
