As soon as an employee verbalizes concerns for their own health or that of a family members, please follow the steps below:
- Take the information and fill out the COVID-19 Report Form by clicking on link provided below.
- Email a copy of the report to Kelly Moreno, RNC Clinic Manager (email@example.com) AND Mario Carmona, Director of Benefits (firstname.lastname@example.org).
- Send employee home immediately.
- Make employee aware that they will be contacted by the Employee Health Clinic and Absence Management.
- The SISD Employee Health Clinic will contact the employee by phone with further instructions (i.e. isolation, testing, etc.).
- Employee Benefits and Absence Management will contact the employee by phone to provide assistance with benefits and leave information.
- Absence Management will contact Campus/Dept. Administration to follow up with staff status related to employee absence and or leave.
