• COVID-19 Reporting

    As soon as an employee verbalizes concerns for their own health or that of a family members, please follow the steps below:

    1. Take the information and fill out the COVID-19 Report Form by clicking on link provided below.
    2. Email a copy of the report to Kelly Moreno, RNC Clinic Manager (kmoren06@sisd.net) AND Mario Carmona, Director of Benefits (mcarmo02@sisd.net).
    3. Send employee home immediately.
    4. Make employee aware that they will be contacted by the Employee Health Clinic and Absence Management.
    5. The SISD Employee Health Clinic will contact the employee by phone with further instructions (i.e. isolation, testing, etc.).
    6. Employee Benefits and Absence Management will contact the employee by phone to provide assistance with benefits and leave information.
    7. Absence Management will contact Campus/Dept. Administration to follow up with staff status related to employee absence and or leave.
