Health Professional & Non-Counseling Therapists Visitors

This application is for visiting Nurses, Health Professionals and Non-Counseling Therapist Professionals that have assigned visits to meet with students or conduct student sessions at SISD campuses.  You may begin your duty assignments after criminal background approval and campus has been notified.  

To apply as a Health Professional or Non-Counseling Therapist, please read the instructions provided below and complete the application process.

Begin the process by filling out the electronic Health Professionals & Non-Counseling Visitors Application.  Once you have completed the form, continue to Step 2 and fill out the SISD Visitors Application.  Do not close the browser until both forms have been completed.
